Student Athlete Code of Conduct Form
This form is required of every athlete who plays a sport at Serra. The signed form will be turned into the student's coach and a copy will be kept on file in the AD office. One form required per year.
Physical & Parent Release Forms
Physicals are required once a year. You must get a physical from your own physician or a primary care center as the Serra sponsored day has passed. Below you will find the required forms for the Athletic Department. You must first fill out the Student Info/ Parent release form. Next, fill out the history form BEFORE your physical (and get a parent signature). Bring the exam form to the doctor. **Turn all 3 completed documents back into Serra HS ***
Serra Athletic Handbook
Below you will find the Serra Athletic Department Handbook. Please read it carefully as it details all of our policies, procedures and philosophy.
NCAA and College Info.
Below you will find two useful documents. The first is the NCAA's publication detailing many requirements necessary to play a sport in college. The second is a useful guide to make sure you are on track for college athletics. Please note that the information presented in these two handbooks changes constantly. Consult the counseling department at Serra and the NCAA website for the latest information.
Athlete & Parent Surveys.
Below you will find the Serra Student Athlete Survey and the Parent Athletic Survey. Student athletes are required to complete the from. Parents should complete the form and will receive one service hour for doing so. Please turn all completed surveys into the main office or Mr. Dunlap.
